Most Remediation Programs Never Confirm the Fix Actually Worked

Mandiant M-Trends 2026 puts mean time to exploit at negative seven days while Verizon's 2025 DBIR finds edge devices take 32 days to remediate. The deeper problem: closing tickets is not the same as closing exposures.

Security teams are marking vulnerabilities as “resolved” at record speed, but a structural failure in most remediation programs means those closed tickets frequently describe open exposures — because the fix was never validated.

That finding is the central argument of analysis published this week drawing on data from Mandiant’s M-Trends 2026 report and the Verizon 2025 Data Breach Investigations Report. The numbers frame a problem that has long existed but is becoming operationally dangerous: the mean time to exploit a disclosed vulnerability is now an estimated negative seven days, meaning adversaries are weaponizing vulnerabilities before patches are formally available. Meanwhile, the Verizon 2025 DBIR puts the median time to remediate edge device vulnerabilities at 32 days. Across that 32-day window, many organizations are operating with no confirmation that the remediation steps they applied actually closed the attack path.

Ticket Throughput Is Not Exposure Reduction

The core problem is a measurement artifact. Vulnerability management programs are evaluated on ticket closure rates. A finding is opened, a fix is assigned, the fix is applied, and the ticket is closed. Nothing in that workflow requires confirming that the underlying security exposure no longer exists.

The ticket says “resolved.” The attack path may still be open.

This disconnect is not theoretical. Patches are often applied partially — a vulnerable library updated in one service but not another, a configuration change applied to production but missed on a staging environment promoted to prod weeks later, a workaround that eliminates the reported attack vector but not a secondary one. None of these partial resolutions would show up as anomalies in a ticket-based metric. All would register as closed.

The issue is compounded by how remediation work actually moves through organizations. A security finding does not land in a queue where engineers are waiting to process it immediately. It competes with existing sprint commitments, maintenance windows, change control processes, and escalation paths that vary by team. A vulnerability assigned to an IT infrastructure team runs on a different schedule than one assigned to a DevOps group, which runs on a different schedule than one assigned to an application engineering team. The organizational drag between ticket creation and validated closure can span weeks — and the Verizon data suggests 32 days is the median, not the outlier.

The Revalidation Gap

The step that most programs omit entirely is post-fix validation: after the remediation action is taken, confirm through retesting that the original attack path is closed and that no new exposure was introduced.

This step is not optional in high-assurance environments. It is standard in regulated industries, penetration testing engagements, and incident response playbooks. But for routine vulnerability management at scale, it rarely happens. Retesting takes time and tooling, and organizations that are already struggling to reduce the median remediation window from 32 days have limited appetite for adding a retest cycle that extends the calendar further.

The result is a class of vulnerabilities that remain exploitable in practice, even though they are closed in the tracking system.

AI Worsens the Exposure Window

The -7 day mean time to exploit figure from M-Trends 2026 represents a shift in attacker capability that makes the validation gap more consequential. When exploitation timelines were measured in weeks after a CVE was published, a 32-day remediation window — however imperfect — at least had a chance of completing before widespread exploitation. When exploitation routinely precedes the patch, that assumption is gone.

AI-assisted exploit development accelerates this further. Adversaries with access to large language models can enumerate variant attack paths, generate proof-of-concept code, and test detections at a pace that was not possible two years ago. A partial fix that eliminates the primary attack path described in a CVE writeup may not eliminate the secondary paths that an AI-assisted adversary discovers in the days after disclosure.

Organizations that cannot confirm their fixes are complete face a specific version of this risk: their patching activity reduces the number of exploitable instances but does not eliminate the exposure, and they have no data telling them otherwise.

What Practitioners Should Do

For vulnerability management and SOC teams, the analysis points to a set of concrete program changes:

Build retest requirements into closure criteria. A ticket should not be closable as “remediated” without evidence of retest — whether that is a scan rescan, a manual validation step, or automated confirmation from a security validation platform. This is a policy and tooling change, not only a process one.

Separate remediation rate from remediation quality in reporting. Ticket throughput tells leadership how fast the team is closing findings. It does not tell them how many exposures remain open in practice. Both metrics need to exist in program reporting.

Prioritize edge devices for confirmed closure. The Verizon 2025 DBIR’s 32-day median specifically covers edge devices — a category that includes VPNs, firewalls, and network appliances that are primary targets in initial access campaigns. Confirmed closure on high-priority edge device findings should be a distinct tracking category.

Test for variant paths, not just the reported vector. When a fix is applied for a named CVE, the retest should include adjacent attack surface, not only the specific condition described in the advisory. AI-assisted adversaries are already doing this; defenders need to as well.

Account for deployment gaps. A patch applied to one instance is not a patch applied everywhere. Validation needs to confirm the fix reached all affected assets, not only that the fix was deployed somewhere.

The underlying problem is not that security teams are slow. It is that most programs cannot distinguish between work that reduced risk and work that closed a ticket.
